Below is a list of errata for the paper:

p. 1012, Table II
There is a typographical error in the fourth equation for the 9/7-F transform.
The fourth equation should read:
    s[n] = s_1[n] + floor{(1/4096)(1817(d[n] + d[n - 1])) + 1/2}
That is, the d_1's should be d's.
